Have you moved?  Will you be moving?

When you’re sharing your new address with family, friends and businesses, please remember to also notify the West Bend School District by completing the WBSD Change of Address Form. Please complete the form as soon as you know you will be moving. Proof of residency can be provided separately, but is required before your address will be updated.  

A new address may affect any or all of the following:

• Transportation - Changes to transportation will not be made until your address has been updated (requires proof of residency).  
• Assigned Elementary School - If your new address is in a different elementary attendance area, we need to know if you wish to remain at your current school or transfer to your new attendance area school.   If you wish to remain at your current school, please compete this Request to Remain Form and submit it to your child's current school.
• Resident District - If your new address is outside the West Bend School District boundaries,  we need to know if you wish to remain at your current school or transfer to your new resident district school.  *It is crucial that you notify us promptly if you’ve moved out of the WBSD and wish to remain due to open enrollment requirements.
• Mailings - Please help us ensure mailings are being sent to the correct address.
