Withdrawal of Consent Form
By state law and school district policy, directory data (student name, address and phone number, date and place of birth, participation in officially recognized activities or sports, weight and height if member of an athletic team, dates of attendance, photograph--still, digital, and video--degrees or awards, and name of school most recently attended) is public information and may be released to the public by the school unless the parent or guardian denies the school permission, in writing, to release all or part of such information.
Please fill out a Withdrawal of Consent form and return it to your child's school if you would like to withdraw consent for this information to be shared.
