Baseball Meeting for Parents/Athletes

East and West Coaches Meeting with Parents and Athletes will be held on Wednesday,

May 6, 2015

*East Baseball Coaches Meeting*  7 PM - Fieldhouse

*West Baseball Coaches Meeting* 7 PM - South Gym
 

Coaches meetings are mandatory for athletes and parents.                                       

If this is your first sport of the season, you will need to register to obtain your participation card.  Registration items can be turned in to the athletic office before school, after school, during lunch, study hall or resource time. 

You will need the following four items to register:

1.      Physical or Alternate year form – Please call 335-5590 if you are not sure of the correct card.  You can see your last physical date on Skyward – family access – health – screening date.  Your physical date on file must be dated April 1, 2013 or later to be valid.

2.      Emergency form

3.      Parent Awareness Statement – read the Athletic Handbook online, print the last page, both athlete and parent must sign

4.      $150 once a year fee

Physical forms, alternate year forms, and emergency forms can be found on the East and West web pages under Athletics –Registration.  Information can be typed in and then printed by using the word version, or you can print a blank form and fill it in by hand using the PDF version

Reminder:  You must have your participation card to show at your first practice.  If you cannot find it, you can purchase a duplicate card for $3.
